Before Your Visit
Complete your patient intake forms — a link will be emailed to you after you schedule
Bring any recent lab work, medical records, or test results on paper, or email them to us ahead of your appointment
Write down your current medications and supplements
Think about your health timeline: when did symptoms start? What have you tried?
Your First Visit
Your initial consultation is a real conversation, not a rushed appointment. Dr. Jeurink will take time to understand your full health history, current symptoms, and goals. She may recommend specific lab testing based on what she hears.
What Happens Next
Once your results are in, you will return for a follow-up where Dr. Jeurink walks you through the findings and presents your personalized care plan, in plain language.

Wear comfortable, loose-fitting clothing
Bring any imaging (X-rays, MRIs) related to your concern on paper or send them to us ahead of your appointment
Your First Visit
We will start with a conversation about your health history and what brought you in. Dr. Jeurink or Dr. Edgren will perform a physical examination and, if appropriate, may perform an adjustment during your first visit. Plan for about 30 to 45 minutes.
Patient Forms
After you schedule your appointment, our office will email you a link to complete your patient intake forms online through our patient portal. If you have questions or need help before your visit, give us a call.

Payment and Insurance
Due to the varied services we offer, insurance coverage varies. We recommend requesting an out-of-pocket cost estimate before your visit — we will provide it in writing.
Chiropractic may be covered under certain plans, including Medicare
Root-cause health services are generally not covered by insurance
We accept cash, check, and all major credit cards
